NHP provides care coordination and support to navigate Health First Colorado (Medicaid) services. NHP is the regional organization for Region 2 in Colorado including Elbert County. NHP works with you to coordinate your care—offering assessments, referrals, and support to help you access your benefits and get the services you need, when you need them.
